Cancellation & Payment Policy.
Cancellations.
We all value our time. Please note when I book an appointment, I specifically set that time aside. If I do not provide me ample notice. I am unable to fill that session. Emergencies will be handled on a case-by-case basis. The policy is as follows.
First no-show or cancellation without 24-hour notice: No fee will be charged and I will contact you for an appropriate time to reschedule.
Second no-show or cancellation without 24-hour notice. The client will be responsible for the full amount of the session that has been quoted to you upon our initial consultation.
Third no show or cancellation without 24-hour notice. The client will be responsible for one and a half times the initially quoted session fee. We will also discuss future appointments and potential termination.
Late Arrival: Clients will be provided 10-15 minutes after the scheduled appointment without prior communication. Should there be no communication and the 10-15 minute mark has passed, the appointment is considered a “no show” or a “missed appointment”. When this occurs the no-show/Cancellation policy above is held into effect and I will follow up with the client appropriately.
Payment Policy.
Sessions are conducted virtually for 45 minutes. Should a session continue 15 minutes passed the allotted session time, the client will be charged accordingly. I DO NOT accept insurance, however, I do offer a sliding scale for clients as needed and this can be discussed on a case-by-case basis.
Payment: Full payment is expected at the time of service. As of 07/07/2023, I will expect payment at the beginning of each session to avoid delayed payment or overdue bills. If you are unable to provide the full amount at the time of the session I am happy to discuss a payment plan set up.
Accepted payment. Credit Card, Pay Pal, HAS/FSA Cards, Cash app and Venmo.
HSA/FSA Disclosure.
JM Nutritional Counseling is not responsible for any of the invoices that are not submitted, unable to be reimbursed, or accepted by any insurance company. It is my job as a provider to simply provide the customer with the requested information. I will not fight with the insurance company on behalf of the customer as I do not accept insurance.
- Each transaction(s) will receive notice in the according application or via electronic receipt. All transactions are recorded, monitored, and reported. Should there be any discrepancy documentation for comparison will need to be provided on both ends.
Failure to pay: for the service(s) performed will result in continued contact until payment is received. Payment will also acquire interest until all debts have been paid. The client will be terminated and as needed reporting/legal action will be taken. One month minimum time period will be allotted.
